Description
Improving the Cancer services supports people affected by cancer (including family members and carers).
People can access support at any point on their cancer journey.
Meeting with a practitioner helps you identify any concerns you have and think about ‘what matters to you?’ The practitioner will support you to think about what the best support for you would be for you and then together create a personalised care plan.
Here are some of the things Improving the Cancer Journey service can offer support with:
- Money or housing worries
- Work
- Caring responsibilities
- Physical concerns
- Emotional Concerns
